Pre-Appointment Guidelines

Pre-Appointment Guidelines

Welcome to Parkside Dental

To ensure that your visit is as smooth and efficient as possible, please review and follow these pre-appointment guidelines. Preparing for your appointment in advance can help us provide you with the best possible care.

1. Review Your Appointment Details

  • Appointment Confirmation: Make sure you have confirmed your appointment date and time. If you haven’t received a confirmation, please contact our office.
  • Arrival Time: Plan to arrive at least 10 minutes before your scheduled appointment to complete any additional paperwork. We are closed from 1PM to 2PM.

2. Update Your Medical History

  • Current Medications: Bring a list of any medications you are currently taking, including dosage and frequency.
  • Medical Conditions: Inform us of any changes in your medical history, including recent surgeries, new diagnoses, or changes in your health status.

3. Insurance and Payment Information

  • Insurance Card: Bring your insurance card and a form of ID.
  • Payment Methods: Be prepared to make any co-payments or payments for services not covered by your insurance. We accept cash, Visa, MasterCard, American Express, Discover, and assigned insurance benefits.

4. Pre-Treatment Instructions

  • Eating and Drinking: Depending on the type of procedure you are having, you may be advised not to eat or drink for a certain period before your appointment. Please follow any specific instructions given to you by our office.
  • Medications: Continue taking your regular medications unless instructed otherwise. If you are scheduled for a procedure that requires sedation, additional instructions may be provided.

5. What to Bring

  • Completed Forms: If you are a new patient, bring the completed New Patient Information Form, Medical History Form, Consent for Treatment Form, and HIPAA Privacy Policy Acknowledgment.
  • X-Rays and Records: If you have recent dental X-rays or records from another dentist, bring them with you or arrange to have them sent to our office in advance.

6. Special Considerations

  • Children: If your child is the patient, bring any comfort items they may need, such as a favorite toy or blanket. Please see our Pediatric Policies
  • Language Assistance: If you need a translator or have specific communication needs, please let us know in advance so we can make the necessary arrangements.

7. Cancellation Policy

  • Notice Requirement: We require at least 48 hours’ notice if you need to cancel or reschedule your appointment. This allows us to offer the time slot to another patient in need of care.
  • Fees: A missed appointment or cancellation fee of $26 per scheduled hour will apply if adequate notice is not provided. Please see our no show policy for more details.
